Steps to take to become State Certified:

Register for a SafeFoodTraining course and exam
Complete the state paperwork
Mail information to the State

Step 1- Attend training and take the exam

Register for an NTC SafeFoodTraining Certification course and exam

Step 2- Complete the paperwork

Complete Initial CFM application form
Include a copy of your exam certificate that you passed the exam
Enclose payment amount as specified on the form made payable to Minnesota Department of Health

Step 3- Mail the paperwork

Send the application form, a copy of your exam certificate and payment to the address on the state application
